Case Manager/ QIDP

As case manager in one of our Community Day Services Program, you will work with a team to assist individuals with developmental disabilities in achieving their personal goals through advocacy, guidance, and coordination with other service entities. You will also co-supervise a team of exceptional professional direct support staff.

Earning Potential:

$20.73 per hour

Schedule:

Typically 8 am - 4:30 pm on Monday through Friday

Requirements:

• Bachelor's Degree in Human Services, Psychology, Sociology, Counseling, Social Work, Education, or a related field (P lease feel free to contact us if you are unsure whether your degree will qualify.)


• Valid/Current Driver's License


• One Year of Experience Working with Individuals with Developmental Disabilities (Experience can be personal, professional, intern/practicum, volunteer-related, or a combination of each.)

Brief Overview of Responsibilities:

• Providing coordination of services (in partnership with another on-site case manager) for the approximately 30 individuals enrolled in the program.


• Responsible for writing service plans, daily and monthly note summaries in a timely manner.


• Responsible for supervising the work of the direct support professionals at the program.


• Assist staff in planning/organizing community and program activities for participants based on their interests.


• Must be committed to improving the lives of individuals with developmental disabilities.
