Care Coordinator
LaSalle Network is hiring a passionate Care Coordinator to support aging and vulnerable populations across the Chicago community. What’s the job?
- Conduct client assessments and develop care plans
- Provide ongoing case management as a Care Coordinator
- Coordinate referrals and connect clients to resources
- Maintain accurate client documentation and records
- Ensure compliance with program requirements
- Collaborate with internal teams and community partners
- Travel locally to meet with clients
- Participate in trainings and maintain certifications
- Support continuous improvement initiatives
- Communicate with clients, families and stakeholders
What’s needed?
- Bachelor’s degree in a related field
- Strong communication and interpersonal skills
- Ability to manage multiple clients as a Care Coordinator
- Experience with case management systems and data entry
- Critical thinking and problem-solving skills
- Valid driver’s license and insurance
- Willingness to travel locally
- Healthcare or social services experience preferred
