A-Line Staffing is now hiring a Case Management Coordinator in Illinois. This position offers competitive pay and the opportunity to support members through telephonic and field-based care coordination in a hybrid setting.

Case Management Coordinator Compensation

Pay Rate: Up to $27.70/hour

Case Management Coordinator Highlights

Location: Illinois (Must reside in the state of Illinois)
Assignment Type: Contract (Up to 90 days — no extension expected)
Work Setting: Hybrid (Work From Home with local field travel within 1 hour of home)

Schedule

Monday – Friday: 8:00 AM – 5:00 PM CST

Additional Information

• Local field travel required within approximately 1 hour of residence
• Mileage reimbursement available for approved travel
• Caseloads range from 250–500 members , based on complexity
• Training provided with structured class start date
• Supports members enrolled in Medicare and Medicaid programs
• Nonclinical role
• Must reside in Illinois

Case Management Coordinator Responsibilities

• Coordinate healthcare services to support member care plans
• Assist members with appointment scheduling and benefit navigation
• Educate members on healthcare resources and available services
• Collaborate with providers, caregivers, and community organizations
• Support members with social determinants of health needs
• Maintain accurate documentation and case records
• Utilize care management tools and internal systems
• Conduct telephonic outreach and in-person visits as required
• Promote member engagement and continuity of care

Case Management Coordinator Requirements

• Bachelor’s degree required in one of the following:
• • Behavioral Health
  • Social Work
  • Psychology
  • Counseling
  • Marriage and Family Therapy
  • Nursing
  • Human Services
    OR non-licensed Master’s degree in a related behavioral health or human services field
• Verifiable High School Diploma or GED required
• Minimum 2 years of experience in behavioral health, social services, or related field
• Ability to work independently in a remote environment
• Strong communication, organizational, and problem-solving skills
• Proficiency with Microsoft Office (Word, Excel, Outlook, PowerPoint)
• Ability to navigate multiple computer systems

Preferred Qualifications

• Case management or discharge planning experience
• Managed care experience
• Experience working with diverse or high-risk populations
• Experience coordinating care for medically complex members
