Primarily responsible for providing direct client care activities under the direction of the supervisor as evidenced by performing the following duties. 

ESSENTIAL DUTIES AND RESPONSIBILITIES  include the following.

  • Administer direct care under the supervision of the supervisor based on demonstrated competence.
  • Demonstrate organizational abilities.
  • Observe physical and/or mental changes and signs and symptoms which may be indicative of change in client status and reports change to supervisor.
  • Implement nursing care plan and evaluates client response in the assisted living setting.
  • Maintain awareness of and implements nursing policies and procedures.
  • Serve as manager on duty in the absence of the Community Director and Nursing Supervisor.
  • Manage CNA’s and support staff when Community Director and Nursing supervisor are away from the facility including correcting, directing, and educating staff as well as discipline enforcement.
  • Demonstrate the knowledge and skills necessary to provide care appropriate to the age and condition of the client.
  • Demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the client's requirements relative to his/her age, specific needs and provides the care needed.
  • Record physical and/or mental changes and signs and symptoms which may be indicative of change in client status.
  • Document care given on a daily basis and incorporates it into the client's chart.
  • Monitor resident self-administration of medication and administer medication following policy procedure on medication. Supervises and performs pharmacy responsibilities. Supervises set up of weekly mediplanner.
  • Demonstrate necessary skills and knowledge to provide care for clients.
  • Utilize principles of aseptic practice in varying situations.
  • Recognize and initiate corrective action.
  • Anticipate the need for specialized equipment and supplies. Assures equipment if functioning properly before the need for use.
  • Utilize principles of proper and safe body alignment in positioning clients.
  • Document and accurately account for all equipment and supplies.
  • Demonstrate accurate knowledge and utilization of skin protocols.
  • Effectively monitors and communicates assessment on clients.
  • Demonstrate ability to handle emergency response system and directs clients, staff to emergency location.
  • Respond to personal emergency response system and directs emergency personnel.
  • Responsible for building keys.
  • Interpret the fireboard when alarm and directs emergency personnel to proper location.
  • On call in Supervisor/Manager absence.
  • Effectively uses verbal, nonverbal, and written communication skills to maintain cooperative and professional relationship.
  • Communicate significant changes in the client’s condition and needs to the supervisor.
  • Establish and maintain cooperative relationships.
  • Assist the supervisor in identifying client problems which require further management.
  • Communicate effectively; tact, poise, emotional stability, integrity, initiative, dependability, good judgement.
  • Adapt to changes in a positive, professional manner.
  • Develop and maintain good communication and rapport with the health care team.
  • Demonstrate a positive attitude regarding quality management.
  • Present a positive guest relation attitude to both the public and employee populations.
  • Represent the work location to all customers in a courteous, professional manner.
  • Promptly and courteously answers telephone and takes information messages.
  • Greet and assist all visitors in a courteous manner.
  • Respect the private nature and confidentiality of all client’s and employees.
  • Dress and conduct self in a professional manner at all times.
  • Demonstrate an ongoing commitment to serving the clients as the most important part of the health care chain.
  • Recognize the importance of teamwork.

  • Perform other duties as assigned.

Requirements

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

Bachelor's degree (B. A.) from four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience. 

LANGUAGE SKILLS

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public. 

MATHEMATICAL SKILLS

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry. 

REASONING ABILITY

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. 

CERTIFICATES, LICENSES, REGISTRATIONS

Graduate from an accredited Licensed Practical Nurse or Registered Nurse program. Must hold a current Illinois license and be CPR certified.

The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, and distance vision. Must meet employment physical and have a tuberculin skin test annually.
