JOB SUMMARY
The CST (Community Support Team) Case Manager provides intensive, community-based mental health services to adults experiencing serious mental illness and co-occurring disorders. This position focuses on stabilization, recovery, and independent living through continuous assessment, individualized care planning, and coordinated service delivery. The CST Case Manager engages clients in their natural environments—home, community, or clinical settings—to promote wellness, reduce hospitalizations, and ensure access to necessary supports and resources.
ESSENTIAL JOB DUTIES
- Provide community-based case management and outreach to clients, maintaining approximately 70% field-based and 30% in-office service delivery.
- Be available on a 24/7 rotating schedule to provide crisis intervention and on-call support.
- Conduct comprehensive assessments and develop individualized treatment plans (IM+CANS) in alignment with client goals and CST program standards.
- Collaborate with psychiatrists, therapists, nursing staff, and community providers to ensure continuity of care and coordinated treatment.
- Facilitate hospital discharge planning, linkage to outpatient services, and follow-up to reduce readmissions.
- Assist clients with housing applications, benefits navigation, vocational support, and daily living skills to promote independence.
- Maintain a productivity standard of 30 billable hours per week, ensuring documentation is completed within required timeframes.
- Prepare and submit accurate clinical documentation (e.g., HRAs, Crisis Plans, progress notes, treatment updates) in accordance with agency and DMH requirements.
- Participate in team meetings, case consultations, and interdisciplinary treatment planning to review client progress and barriers.
- Uphold compliance with billing, confidentiality, and quality assurance standards.
- Participate in professional development and training as required by the agency and funding sources.
- Perform other related duties as assigned to support departmental and organizational goals.
QUALIFICATIONS
- Bachelor’s degree in Human Services, Social Work, Psychology, Counseling, or a related behavioral health field required.
- Minimum of 2–3 years’ experience working with adults in a community mental health, case management, or CST setting preferred.
- Familiarity with DSM-5 diagnostic criteria and ASAM levels of care for substance use disorders.
- Strong knowledge of community resources, psychiatric rehabilitation, and recovery-oriented service models.
- Demonstrated ability to engage clients in crisis, manage challenging behaviors, and support complex needs.
- Excellent written and verbal communication, documentation accuracy, and time management skills.
- Valid Illinois driver’s license and reliable vehicle required for community-based travel.
- Bilingual (English/Spanish) proficiency preferred.
- Proficient in Microsoft Office and EHR systems (e.g., SmartCare, Credible, or equivalent).

- Strengthen professional skills through quarterly training on evidence-based practices and community mental health standards.
- Achieve 90% or higher documentation compliance across all assigned cases within the first six months.
- Establish and maintain a minimum of 10 active community partnerships for client resource referrals.
- Implement client-centered interventions that demonstrate measurable progress toward recovery goals.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
- Work performed in office, community, and client home environments.
- Requires moderate physical activity, including the ability to lift or carry 10–25 pounds and conduct regular field visits.
- Occasional exposure to environmental or situational risk during community outreach or crisis response.

Occupational Risk for Exposure to TB:
There is an occupational risk for exposure to TB, as this position involves extensive community and patient contact. Employees in this role are required to comply with annual TB testing as part of workplace health and safety protocols. The only acceptable exceptions to this requirement are documented medical contraindications or religious objections. Employees who decline TB testing will be required to wear a mask when deemed necessary.
