Position: Home Health Occupational Therapist (OT)

We're looking for a dedicated full-time OT to provide personalized, one-on-one care to patients in their homes. This role offers the autonomy to create tailored treatment plans and enhance patients' independence and quality of life. Enjoy flexible scheduling!

What You'll Do:

• Empower Daily Living: Conduct thorough patient evaluations and develop individualized treatment plans based on physician orders.
• Restore Independence: Assist patients in improving daily living activities and functional mobility through targeted interventions.
• Optimize Home Environments: Provide hands-on therapy, including adaptive equipment training and home modification recommendations, to enhance safety and accessibility.
• Educate and Support: Provide comprehensive education to patients and families on treatment procedures and home exercise programs.
• Drive Progress: Continuously reassess and modify treatment plans to maximize patient outcomes.
• Collaborate for Success: Work closely with physicians and a multidisciplinary team to ensure seamless, coordinated care.
• Maintain Excellence: Ensure accurate and timely documentation using Epic EMR.

What You'll Bring:

• Bachelor’s Degree in Occupational Therapy (Required).
• Active Illinois Occupational Therapist License in good standing (Required).
• Valid Driver’s License (Required).
• A passion for home health and a patient-centered approach.

· Occupational Therapists with a passion for home health are encouraged to apply— training available!

Schedule & Flexibility:

• Monday – Friday, (8:00 AM – 4:30 PM) with occasional weekend and holiday rotation.
• 4-10s available!
• Centralized territories to minimize travel.

Compensation & Benefits:

• Competitive Salary: $65,000 - $110,000 per year (commensurate with experience).
• $2,500 Sign-On Bonus! (Paid in the 2nd pay period).
