Benefits and Features :

• $180,000 - $240,000 / year
• up to 11k in bonuses /y r
• 1 month and 2 weeks off per year work only 10 months and 2 weeks a year
• NHSC loan repayment sites, score is 19

Description:

• 3 patients per hour seen 80% are adults, 20% under 18 yrs old
• Mon-Fri 8-5, no call
• 4 days a week on site and 1 day week from home.can work 2-4 days per week from home (telehealth) if desired, but need at least 1 day a week on site in Chicago

Profile we need:

• Must speak Spanish at a 7 out of 10 score or better, many patients are older with hearing issues, and they only speak Spanish
• Someone aligned with their mission and values
• Someone good with EHR
• Must work well in teams PCMH model MAs, Receptionist, Care Coordinators, and Care Managers
