- Provide safe, compassionate nursing care to residents based on individualized care plans
- Administer medications and treatments accurately
- Monitor residents’ health and report changes to the RN, provider, and families as needed
- Document assessments, care, and observations in the EHR
- Support and guide caregivers and CNAs during daily care
- Communicate effectively with physicians, families, and the care team
- Participate in care plan meetings, quality initiatives, and state surveys
- Follow infection control and safety procedures
- Complete other duties as assigned
What You’ll Need:
- High school diploma or GED
- Active, LPN license in the state of Illinois
- Supervisory experience preferred
- Experience in long-term care or a similar setting preferred
- Compassionate, dependable, and strong communication skills
- Ability to maintain resident confidentiality
- Ability to lift, move, and assist residents safely
- Must pass a criminal background check and required health screenings (TB and others)
- Ability to read, write, and understand English
