Job Overview:

- Duration: 13 weeks with possible extension
- Shift: Days (8:00 AM – 5:30 PM, 40-hour guarantee)
Responsibilities:
- Provide patient care through the nursing process (assessment, planning, implementation, evaluation).
- Supervise CNAs and coordinate with other healthcare workers.
- Conduct comprehensive initial and follow-up assessments (physical, psychosocial, environmental).
- Initiate and update Plans of Care; establish discharge plans.
- Complete OASIS documentation.
- Educate patients, families, and caregivers.
- Ensure compliance with federal, state, and agency regulations.
- Promote safety, privacy, and confidentiality.
- Perform telehealth/virtual visits as needed.
Requirements
Requirements:
- Home Health experience (required; strong Med Surg with some Home Health considered).
- Valid driver’s license & car insurance (required).
- Illinois RN license (active at submission).
- BLS certification (required).
- COVID & Flu vaccination (declinations not accepted).
- OASIS & Start of Care experience (strongly preferred).
- Local candidates prioritized.
