Seven Healthcare are seeking an experienced RN - Hospice for a travel assignment in Green Bay, Illinois. This role involves working as a Hospice Liaison in a flexible work environment across multiple locations in the Northern area, including Green Bay, Oshkosh, Manitowoc, Sheboygan, and Grafton. The ideal candidate will have hospice experience and be comfortable with transitioning patients to hospice care.

Key Responsibilities of the RN - Hospice - Travel

• Monitor queue and consult with patients referred from hospitals or SNF for hospice.
• Provide assessments and recommendations on proper hospice settings.
• Admit to hospice services in hospital settings when necessary.
• Collaborate with physicians and provide education on hospice benefits and philosophy.

Requirements of the RN - Hospice - Travel

• BLS, Driver's License, Auto Insurance, and Vehicle required.
• Minimum 2 years of hospice experience preferred.
• EPIC experience required.
• Comfortable with end-of-life symptom management and medication.
