?? This is a home health role-travel between patient homes is required.

?? What You'll Do

• Evaluate and treat patients with speech, language, and swallowing disorders

• Develop and implement individualized plans of care

• Perform OASIS assessments and ongoing re-evaluations

• Collaborate with physicians and interdisciplinary teams

• Educate patients and caregivers for continued success at home

• Ensure accurate, timely documentation

? Master's degree in Speech-Language Pathology

? Active Illinois SLP license (unrestricted)

? At least 1 year of clinical experience

? Valid driver's license + reliable transportation

? Willingness to travel within coverage area

You will report to the Branch Director, Clinical Manager, Clinical Supervisor or designee.
