Home Health RN Job Details

• Location: Oak Brook, IL and surrounding areas (Chicago metro – Illinois nursing jobs)
• Assignment Type: Locum tenens (Travel RN contract)
• Start Date: August 25, 2025
• End Date: November 22, 2025
• Duration: 13-week contract
• Shift: 8-hour day shifts
• Hours per Week: 40
• Estimated Weekly Salary: $2,870 - $2,950 per week
• Setting: Home Health (100% remote after 2-day onsite orientation)
• Patient Load: 20-30 patients per week; 6 routine visits per day

Registered Nurse Qualifications

• Active Registered Nurse (RN) license in Illinois
• Active BLS certification (AHA required)
• Minimum 1 year of recent Home Health RN experience
• Wound care experience (wound vacs, pleural drains, ostomy care)
• Proficient in peripheral blood draws, central line care, and IV administration (elastomeric, IV pumps)
• Strong assessment and patient education skills
• Experience with electronic health record documentation (EPIC preferred)
• Reliable transportation, valid driver’s license, and auto insurance (required for travel nursing jobs)

RN Case Manager Responsibilities

• Conduct home health visits for patients with complex comorbidities (CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, post-surgical, wound care, and more)
• Educate patients and caregivers on medication management, disease processes, and self-care techniques
• Perform advanced wound care, infusion therapy, line maintenance, ostomy care, and related nursing procedures
• Teach and support patients and families in performing necessary care tasks
• Complete accurate and timely clinical documentation using EPIC
• Participate in virtual meetings and collaborate with interdisciplinary healthcare teams
• Travel within the Oak Brook and greater Chicago area to deliver patient-centered care
