Seven Healthcare are seeking an experienced RN for a home health travel assignment in West Allis, Illinois. This role focuses on skilled nursing and therapy services within a home setting. As a Medicare-certified agency, the team supports patients with conditions such as CHF, COPD, diabetes, and post-surgical care, among others.

Key Responsibilities of the RN - Home Health - Travel

• Provide skilled nursing care and therapy services to home-based patients
• Conduct clinical assessments and manage medication
• Deliver wound care and manage fall risk or deconditioning
• Utilize OASIS documentation and support a productivity-based model
• Collaborate with CNAs, a scheduling team, and clinical leadership

Requirements of the RN - Home Health - Travel

• Active RN license in Wisconsin
• BLS certification
• Valid driver's license and reliable transportation
• Associate or Bachelor's degree in Nursing
• 2+ years of experience preferred, home health experience strongly preferred
