The role is community-based and centered on meeting people in their actual living environments to deliver compassionate, person-centered care. Funded to serve patients who may not fit within the traditional medical model, this position is a part of the care team which includes medical providers, primary care teams, and community partners to improve health outcomes and reduce disparities among unsheltered and housing-insecure individuals. The role also includes advocating for patients’ social and environmental needs as they arise. Collaboration with health center sites, primary care providers, local health departments, and community-based organizations is essential to provide wraparound services and build a foundation of trust that supports ongoing care for socially vulnerable populations. - Explain program requirements and guidelines to the general public.
- Develop community resource materials that are up to date and reflective of the local community.
- Conduct comprehensive interviews and intake assessments to determine the needs of individuals to be
- Assist patients in setting and achieving personal goals in person, by phone or in group setting.
- Through skill building interventions and other supportive interventions, assist patients in effectively
- Connect patients to community resources including but not limited to housing, medical care, education,
- Assist patients with transportation to medical and other related appointments.
- Prepare and maintain written records as needed by the program.
- Participate in all required administrative and training meetings in order to ensure adherence to policies
- Complete other duties as assigned.
- Any one or combination totaling three (3) years (36 months) from the categories below:
- Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent:
- 30 semester hours equals one (1) year (12 months)
- Associate’s Degree (60 semester hours) equals eighteen months (18 months)
- 90 semester hours equals two (2) years (24 months)
- Bachelor’s Degree (120 semester hours) equals three (3) years (36 months)
- Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.
- Working knowledge of human nature.
- Ability to interview individuals to determine their physical, social, and/or possible psychological needs.
- Ability to organize, prepare, and maintain records, reports, files, and correspondence.
- Ability to obtain services necessary to meet participants' needs.
- Ability to establish effective working relationships with program participants and staff.
