Description

This position provides direct outreach and services to individuals living on the streets, in encampments, or experiencing housing insecurity. The role is community-based and centered on meeting people in their actual living environments to deliver compassionate, person-centered care. Funded to serve patients who may not fit within the traditional medical model, this position is a part of the care team which includes medical providers, primary care teams, and community partners to improve health outcomes and reduce disparities among unsheltered and housing-insecure individuals. The role also includes advocating for patients’ social and environmental needs as they arise. Collaboration with health center sites, primary care providers, local health departments, and community-based organizations is essential to provide wraparound services and build a foundation of trust that supports ongoing care for socially vulnerable populations.

Examples of Duties

Support - Direct Patient Interaction 100%

• This position works in the streets, encampments and clinic to assess patient needs, provide direct
  
  

support, and connect individuals experiencing homelessness or housing insecurity to medical and

community resources.

• Explain program requirements and guidelines to the general public.
• Develop community resource materials that are up to date and reflective of the local community.
• Conduct comprehensive interviews and intake assessments to determine the needs of individuals to be
  
  

served by the program.

• Assist patients in setting and achieving personal goals in person, by phone or in group setting.
• Through skill building interventions and other supportive interventions, assist patients in effectively
  
  

utilizing a wide spectrum of resources, managing symptoms of their illness, and developing daily living

skills to gain greater independence, employment and other meaningful activities.

• Connect patients to community resources including but not limited to housing, medical care, education,
  
  

vocational training, and additional recovery resources / meetings.

• Assist patients with transportation to medical and other related appointments.
• Prepare and maintain written records as needed by the program.
• Participate in all required administrative and training meetings in order to ensure adherence to policies
  
  

and protocols.

• Complete other duties as assigned.
  
  

Qualifications

Credentials to be Verified by Placement Officer

• Any one or combination totaling three (3) years (36 months) from the categories below:
• Coursework in social work or behavioral sciences, or closely related field as measured by the following conversion table or its proportional equivalent:
• 30 semester hours equals one (1) year (12 months)
• Associate’s Degree (60 semester hours) equals eighteen months (18 months)
• 90 semester hours equals two (2) years (24 months)
• Bachelor’s Degree (120 semester hours) equals three (3) years (36 months)
• Progressively more responsible work experience in social service/ counseling at a paraprofessional level under qualified supervision.
  
  

Knowledge, Skills And Abilities (KSAs)

• Working knowledge of human nature.
• Ability to interview individuals to determine their physical, social, and/or possible psychological needs.
• Ability to organize, prepare, and maintain records, reports, files, and correspondence.
• Ability to obtain services necessary to meet participants' needs.
• Ability to establish effective working relationships with program participants and staff.
  
  

Condition of Employment

Pursuant to the State Universities Civil Service System, an out-of-state resident who is hired into this position must establish Illinois residency within 180 calendar days of their start date.
