Position Summary
This position is responsible for providing intense case management services to residents of the Deborah's Place Rapid Rehousing Program, including: intake and assessment with new residents, collaborative development of individual service plans with residents, crisis intervention and linkages to resources/referrals. The Lead Case Manager works to assist residents in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals with the objective of residents being prepared to take on full responsibility for market-based rent and leases within a 12-month period.
Valid Illinois driver's license, proof of insurance, and must have own vehicle. Mileage reimbursement provided.
We offer supervised clinical hours towards LCSW and LSW certifications.
Essential Duties and Responsibilities
Rapid Rehousing Case Management Services
- Provide case management services including intakes, assessments, service planning, staffing, individual counseling, referral, and advocacy.
- Provide crisis intervention in mental health and/or medical emergencies. Provide on-call assistance with after hour and weekend emergencies.
- Assist with applications and advocacy related to public benefits.
- Plan and implement life skills individual and group activities.
- Work with residents to achieve their goals related to housing, income and health with the objective of ensuring that residents are prepared to take full responsibility for rent and lease within a 12-month period.
- Assess and respond to residents' motivation and readiness using motivational interviewing skills.
- Engage in clinical supervision.
- Work with residents immediately to address issues in order to prevent eviction.
- Participate in regular meetings with other providers in the Expedited Housing Initiative (EHI) Rapid Rehousing Program.
- Provide on-call assistance with after hour, holiday and weekend emergencies.
Record Keeping and Data Quality
- Prepare and maintain current tenant files.
- Enter all services and required information into HMIS and other service and outcome tracking databases as required.
- Work collaboratively with the Program Data Manager to ensure data quality.
Qualifications and Requirements
Education: Bachelor's Degree in Social Work or related field.
Licensure and/or Certification: None required.
Job Experience: 2 - 3 years of case management experience, including in the field of rapid rehousing or in other models in which financial assistance and services are time-limited.
Special Knowledge & Skills: Knowledge of issues surrounding homelessness, mental illness and substance use; knowledge of the Rapid Rehousing model; proficiency in Microsoft office software, good oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner. Experience in critical time intervention a plus. Valid Illinois driver's license, proof of insurance and must have own vehicle.
