SUMMARY
Provide interim case management services for DCFS youth in care through effective care coordination; ensuring timely access and receipt of quality healthcare services.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

Conduct a diligent search of historical and current medical documentation.

Educate youth in care and caregivers on the HealthWorks Network of services and providers.

Ensure timely scheduling of CHE appointment within 7 days or sooner (depending on the benchmark) of speaking to the foster parent/youth in care or group home. Summary

The Interim Care Coordinator I provides interim case management services for DCFS youth in care by ensuring timely, coordinated access to quality healthcare services. This role supports caregivers and youth throughout the HealthWorks network, ensures compliance with state benchmarks, and maintains accurate and timely documentation across all case activities.

Essential Duties and Responsibilities

• Conduct a thorough search of historical and current medical records to gather documentation for each case.

• Educate youth in care and their caregivers about the HealthWorks network and available providers.

• Schedule Comprehensive Health Evaluations (CHE) within 7 days (or sooner, depending on benchmarks) of initial caregiver/youth contact.

• Ensure that at least 90% of assigned cases receive a completed CHE.

• Monitor the Day-15 report and investigate cases with no scheduled CHE, troubleshooting to avoid delays.

• Ensure 90% of CHEs are completed within 21 days of temporary custody.

• Provide transportation to youth in care when needed—particularly when caregivers have three or more children or are unavailable—to ensure CHE appointments are kept within timelines.

• Ensure 98% of youth in care are enrolled with a primary care physician (preferably within the HealthWorks network) before case closure.

• Confirm primary dental care enrollment prior to case closure.

• Complete and file all necessary documents according to HealthWorks standards for case closure.

• Prioritize and submit cases with completed CHEs to the QI nurse within 48 hours for summary completion.

• Document all healthcare services in SACWIS and the internal database per required timelines.

• Attend all required trainings and team meetings.

• Perform additional duties as assigned.

Education and Experience

• Bachelor’s degree required.

• Minimum two years of related experience and/or training, or equivalent combination of education and experience.

Language Skills

• Ability to read and interpret documents such as safety rules, procedure manuals, and operating instructions.

• Proficient in writing routine reports and professional correspondence.

Mathematical Skills

• Ability to calculate percentages, ratios, proportions, and apply basic algebra and geometry concepts.

Reasoning Ability

• Ability to solve practical problems and manage multiple variables in unstructured situations.

• Capable of interpreting written, oral, diagram, or schedule-based instructions.

Certificates, Licenses, Registrations

• Valid driver’s license.

With additional education, training, and experience, an Interim Care Coordinator may be promoted to Interim Care Coordinator II.
