Certified Rehabilitation Recovery Specialist
We are seeking a dedicated and compassionate CRSS to join our team. In this vital role, you will coordinate and oversee client care plans, ensuring individuals receive the appropriate support and resources to achieve their goals. Key Responsibilities:
- Assess clients' needs and develop personalized care plans
- Coordinate services and resources across multiple agencies and providers
- Monitor client progress and adjust care plans as necessary
- Advocate for clients to ensure they receive appropriate support
- Maintain accurate and detailed case records and documentation
- Collaborate with team members and external stakeholders to optimize client outcomes
- Provide guidance and support to clients navigating complex systems
Skills and Qualifications:
- Bachelor's degree in social work, psychology, healthcare, or related field (Master’s preferred)
- Proven experience in case management, social services, or related roles
- Strong interpersonal and communication skills
- Excellent organizational and time-management abilities
- Ability to handle sensitive information with confidentiality and professionalism
- Knowledge of community resources, healthcare systems, and social services
- Problem-solving skills and the ability to work independently and as part of a team
