We are seeking a highly motivated and results-oriented Project Manager to support the planning, implementation, and ongoing management of Medrina's participation in the ACO REACH/LEAD ACO model focused on high-needs populations across post-acute, long-term care, skilled nursing, assisted living, and community-based settings. The Project Manager will serve as the central coordinator for all LEAD ACO startup activities, ensuring alignment across clinical operations, value-based care, quality, technology, finance, compliance, provider engagement, care management, and external partners. This individual will work closely with executive leadership, physician leaders, payor partners, technology vendors, and operational teams to build scalable infrastructure that improves patient outcomes while achieving financial and quality objectives.

This is a full-time role operating from central or eastern regions of United States during standard business hours Monday - Friday (from your home office remotely) with as-needed and quarterly travel in United States. This is not a flexible schedule opportunity. The candidate must reside in central or eastern region of United States. This role offers an a nnual salary of $120,000. Job Responsibilities:

ACO Startup & Implementation

  • Lead day-to-day management of the LEAD ACO implementation plan.
  • Develop and maintain detailed project plans, workstreams, milestones, dependencies, and risk mitigation strategies.
  • Coordinate startup activities across clinical, operational, financial, compliance, technology, and care management teams.
  • Monitor project timelines and escalate barriers to executive leadership.
  • Facilitate implementation meetings and maintain project documentation.
  • Track readiness activities associated with CMS LEAD ACO requirements.

Program Development

  • Assist in developing operational workflows supporting: Attribution management, Care coordination, Transitional care management (TCM), Chronic care management (CCM), Behavioral health integration, Advanced care planning, Hospital-to-SNF transitions, Community-based care programs
  • Coordinate development of standard operating procedures and playbooks.
  • Support creation of care pathways for high-risk and high-needs populations.
  • Assist with implementation of quality improvement initiatives.

Provider & Clinical Operations

  • Partner with physician leadership to support provider engagement and adoption.
  • Coordinate communication between Medrina providers, APPs, care coordinators, and partner organizations.
  • Track provider participation, education, and training initiatives.
  • Support development of provider performance scorecards and quality reporting.
  • Coordinate implementation of clinical interventions aimed at reducing avoidable hospitalizations and readmissions.

Value-Based Care & Population Health

  • Support operational execution of Medrina's value-based care strategy.
  • Assist in development of risk stratification workflows.
  • Coordinate outreach initiatives targeting high-risk beneficiaries.
  • Track performance against utilization, quality, and cost benchmarks.
  • Monitor care gap closure initiatives and intervention programs.
  • Support implementation of social determinants of health (SDOH) initiatives.

Technology & Data Integration

  • Coordinate implementation of technology solutions supporting the LEAD ACO.
  • Work with vendors to ensure integration of: EMRs, HIE platforms, Care management systems, Analytics platforms, Population health tools, Reporting dashboards
  • Assist with data validation and reporting accuracy.
  • Support development of executive dashboards and operational reporting.

Quality & Compliance

  • Assist in development of CMS-required policies and procedures.
  • Coordinate quality reporting activities.
  • Monitor compliance with CMS ACO regulations and reporting requirements.
  • Support audit preparation and regulatory readiness activities.
  • Maintain documentation supporting governance and operational compliance.

Financial & Performance Management

  • Support tracking of ACO financial performance.
  • Assist with budget management for implementation initiatives.
  • Monitor project ROI and performance outcomes.
  • Coordinate reporting related to shared savings and value-based arrangements.
  • Support development of business cases for new initiatives.

Stakeholder Management

  • Serve as liaison between: Executive leadership, Clinical leadership, Care management teams, Technology vendors, Health plans, Hospitals, Skilled nursing facilities, Community providers
  • Prepare executive-level presentations and status updates.
  • Facilitate steering committee and governance meetings.

Job Requirements:

  • Bachelor's degree in Healthcare Administration, Business Administration, Public Health, Nursing, or related field.
  • 5+ years of healthcare project management experience with ACOs, Medicare Shared Savings Program (MSSP), ACO REACH, Medicare Advantage and Value-based care programs.
  • PMP certification.
  • Experience in post-acute care, skilled nursing, hospital medicine, or population health.
  • Experience leading complex cross-functional healthcare initiatives.
  • Advanced project management and organizational skills and proficiency with project management tools and reporting systems.
  • Familiarity with CMS quality measures and risk adjustment programs.
  • Experience working with healthcare analytics and reporting platforms.

Physical Requirements:

  • This is a remote, work from home position which handles sensitive, and confidential information. Work must be performed in a secure and private setting.
  • Communicating via virtual online meetings daily with reliable internet connectivity with camera on as well as being responsive in a timely manner during work hours via email, MS Teams and phone are required.
  • Travel in United States for on-site visits and company meetings is required.
  • Ability to sit, stand, and walk for extended periods.
  • Ability to lift and move up to 25 pounds.
