RN Navigator - Structural Heart Coordinator (Urbana)

Overview

The HVI Structural Heart RN will assist patients in navigating through the referral, diagnostic, pre-procedure, and post-procedure process for surgical and medical management of structural heart conditions. Applies the nursing process through assessment, planning, implementation and evaluation of the patient in order to provide the optimal level of patient care.
Qualifications

Certifications: Basic Life Support (BLS) within 30 days - American Heart Association (AHA); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), Education: , Work Experience:
Responsibilities

Assists patients in understanding diagnosis, treatment options and resources available internal and external to the organization. Manages patient’s necessary appointments and orders pre and post procedure Work collaboratively with structural heart providers and coordinator to develop a cohesive and efficient process for the treatment and evaluation of patients referred to the structural heart clinic Provide education and informational resources to patient to optimize continuous and comprehensive care. Manage referrals from physician, assess clinical data needed for structural heart procedures following QHR (quality) best practice standards Collaborates with structural heart team including the Coordinator, Cardiothoracic Surgeons, and Cardiologists for planning and scheduling procedures Data uploads with 3 rd party vendors for evaluation pre procedure Provides patient and family education and patient assessments pre and post procedure Adheres to Registry requirements including abstracting and documentation of required elements

Compensation and Benefits

The compensation range for this position is $32.21per hour - $55.4per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate’s experience, qualifications, location, training, licenses, shifts worked and compensation model.
