Overview
We are seeking a dedicated and compassionate Case Manager Coordinator to join our team. This vital role involves coordinating services for individuals facing behavioral health challenges, addiction issues, and social service needs. The ideal candidate will possess strong crisis intervention skills, experience in social work, and a passion for supporting vulnerable populations. The position offers an opportunity to make a meaningful impact in the community by providing comprehensive case management and resource linkage.

Duties

• Conduct thorough assessments to identify client needs related to behavioral health, addiction, child welfare, and other social services.
• Develop individualized care plans in collaboration with clients, families, and service providers.
• Provide crisis intervention and crisis management to ensure client safety and stability.
• Coordinate access to services such as Medicaid, Medicare, Section 8 housing, workforce development programs, and childcare resources.
• Utilize motivational interviewing techniques to motivate clients toward positive change and treatment adherence.
• Supervise and mentor junior staff or interns as needed, fostering a supportive team environment.
• Maintain detailed documentation of client interactions, service plans, and progress notes in compliance with agency policies.
• Collaborate with community partners, healthcare providers, and social service agencies to facilitate seamless service delivery.
• Monitor client progress and adjust care plans accordingly to meet evolving needs.
• Stay informed about current policies related to child welfare, Medicaid, Medicare, and workforce development initiatives.

Qualifications:

• Bachelor’s degree in behavioral health or human services (psychology, social work, marriage & family therapy, nursing, counseling, etc.)

OR a non-licensed, master’s-level clinician

• Case management and/or discharge planning required
• Minimum 2 years of experience in behavioral health, social services, or a related field
• Ability to work independently and confidently in a field-based role
• Proficiency with MS Word, Excel, Outlook, PowerPoint, and navigating multiple systems
• Managed care experience
