Physical Therapist - (Home Health PT)
We are seeking a dedicated Physical Therapist to provide home health services in and around St. Charles, IL / Downtown Chicago, IL and surrounding areas within roughly a 20-mile travel radius. The therapist will evaluate and treat patients in their homes, develop individualized care plans, and collaborate with the interdisciplinary care team to support patient recovery and independence.
Commute Range – A 20-mile radius from St. Charles, IL / Downtown Chicago, IL covers all the listed coverages for this role.
- Territory Coverage: Manageable driving radius and Travel reimbursement will be provided as per home health setting.
- Flexible Scheduling – Choose your own hours (Full-time, Part-time, or PRN). 1099 options also available.
- Competitive Compensation – Per-visit rates starting at $65 and up to $115+ per evaluation depending on experience. Additional visit types (Treatment, SOC Oasis, Discharge/Recert) rates will be discussed during the phone interview.
