We serve over 100 residents with dozens of locations throughout the Illinois-Metro East area. With a team of highly trained and dedicated care professional, we offer 24/7 services and choice of the model of home (Community Integrated Living Arrangement or Intermediate Care Facility/Developmental Disability) that best meets a person's preference.

Position Summary:

  • Shift : Full-Time
  • Hours: Monday-Friday; Days + on-call; rotating weekends.
  • On-Call: Provides on-call services to assigned group homes from 3pm-7am Monday thru Friday and scheduled weekends/evenings. Must respond within 15 minutes.
  • Salary : $78,728 annually with Benefits
  • Location: Alton and Swansea RO Admin Office + assigned sites , IL
  • Travel : 25-40% between IL sites to travel to meetings, trainings, or to transport residents.
Reporting to the VP (Vice President) of Residential Options, the Director of Nursing is responsible for providing nursing services to approximately 154 Residents with developmental disabilites or mental illness living in group homes which provide opportunities for them to live in a home-based environment -achieving the highest level of independence possible.

Responsibilities
  • Promote a culture of person-centered services that advances the Company's mission to provide quality care and services in a community-based setting to individuals with disabilites. Ensuring a high rate of resident satisfaction, ensure the health and safety needs of all residents are met by providing nursing services to residents.
  • Recruit, screen, and interview prospective residents as requested. Attend all resident PP/ITP,IHP staffing meeting in assigned homes or send medical summary prior to the meeting.
  • Plan, organize, and supervise the delivery of care to residential residents. Complete annual nursing assessment, quarterly nursing assessment, TD, self-medication assessment and monthly observations, admission, NSP'S, TB signs and symptoms check-list, influenza immunization, tetanus, pneumococcal, and hepatitis series as ordered by physician. Visit assigned Residents no less than bi-weekly to evaluate their current condition and assist in healthcare planning. Complete reports and communicate key information to the VP of Residential Options in a timely, accurate, and comprehensive manner. Ensure that each facility meets state, federal, CARF (Commission, on Accreditation of Rehabilitation Facilities) and any governing agency's regulatory guidelines.
  • Maintain records of assigned Residents for medications, charts, vaccinations, and annual assessments accurately and timely. Review all Physician Order Sheets and Medications Administration Records prior to being placed in the Resident's chart in a timely manner at least monthly. Review and sign incident reports with follow up as needed. Review all medication errors reports, complete medication error form and forward to Public Health, DHS and OIG (Office of Inspector General) according to regulations. Review all labs and forward to the prescribing physician for review. Communicate effectively with physicians and service providers regarding Resident care. Order medical supplies as needed. Assist with and oversee the training of direct care staff on Basic Health and Safety, Medication Administration and other health related topics as required. Provide immunizations and TB screenings to employees and maintain accurate records.
  • Answer calls in a timely manner and provide on-site services as required within 24 hours. Follows through with reported problems and communicates the mto supervisor/primary care nurse. Check nurses' mailbox at group homes weekly.
  • Lead team and place people in positions to succeed. Hire, train, and empower top-notch leaders to effectively oversee their areas of responsibility. Encourage effective outcomes and accountability. Plan, develop, organize, implement, direct, and evaluate assigned sites' activities and performance. Communicate job expectations. Plan, monitor, and appraise job results; evaluate team for continual improvement, efficiency and effectiveness. Initiate, coordinate, and enforce systems, polices, and procedures. Mentor and develop the team through training, problem resolution, and performance evaluation. Conduct monthly nurse/PBL meetings and individual nurse consultations. Provide nurses' monthly on-call duties.
Qualifications
  • Education : Illinois Professional Registered Nurse (RN) License, be certified as a Nurse Trainer by Department of Human Services (or obtain DHS certification within 2 months of starting the job).
  • Experience: 1+ years of supervisory experience. Experience working with people with developmental disabilites or mental illness and two years RN experience helpful but not required.
  • Certifications/Licenses: RN License and Nurse Trainer license from DHS, CPR, Crisis Prevention Institute (CPI), and DSP (Direct Support Professional) training, including medication administration, provided by the company must be successfully completed within the first 4 months to be certified. Passing CPR and CPI training is also required annually.
  • Computer Skills : Proficient with Microsoft Word, Excel, and Outlook.
  • Background Checks : Must pass criminal background check, various State and Federal registry checks, DCFS Abuse and Neglect Tracking System check.
  • Driving : must be at least 21 years or older to drive for Company related travel, pass driving history check from the past 3 years, maintain valid drivers license and use of personal insured vehicle is required.

Physical Demands
  • Sitting : Frequently sitting in the normal course of office-sedentary type work.
  • Standing : Occasionally standing, walking, bending, squatting, reaching, and twisting in the normal course of office-sedentary type work and for training workers.
  • Hearing, Speaking: Frequently listening to and speaking with managers, staff, and residents to communicate about requests, training needs and other concerns.
  • Seeing : Continually visually alert to monitor, to read and write or type documents, observe employee and resident interactions, and communicate with all.
  • Handling : Frequently using hands in writing and typing, and general office work.
  • Movement : Kneeling required to perform CPR practice sessions, complete skills testing and use as required. Occasionally bending, reaching, and twisting; occasionally climbing stairs and/or ladders squatting.
  • Lifting : May need to lift or carry up to 50 lbs. to assist a Resident to their feet from a lying or sitting position, to help with walking or for the Resident to change positions.
