Job Description

The BLOOM Cohort Wellness Coordinator plays a key role in supporting participant success and program effectiveness through cohort outreach, while supporting program management and administrative duties as needed. This position works closely with program leadership and the clinical team to ensure high-quality implementation of the BLOOM Cohort curriculum and delivery of services. The Wellness Coordinator builds strong relationships with participants, supports wellness programming, and ensures that key operational needs related to outreach are met.

Mission & Programming

Free Root Operation is a budding nonprofit that empowers families to disrupt cycles of violence by cultivating their healing, power, and capacity for generational change. We serve single mothers and women, often erased from gun violence advocacy, as a public safety infrastructure. Our work helps participants to heal from harm, break generational cycles of violence, harness their influence as community leaders, and shift the trajectory of their families.

Our flagship initiative, The BLOOM Network, is a fleet of several wellness programs designed to meet our constituents at different levels of their wellness journeys. Our Network currently has over 1,800 registrants in the Chicagoland area. The deepest level of care in the Network is the BLOOM Cohort: a seven-month intensive community violence intervention program that combines individual and family mental health counseling, educational and experiential wellness workshops, healing and wellness education, and community leadership training to produce measurable improvements in participants' hope, energy, mood, and self-worth levels.

Role Responsibilities

Outreach

  • Execute recurring phone, text, email, and GroupMe outreach to participants to help them navigate challenges, provide programmatic guidance, and coach them toward success.
  • Facilitate virtual peer support spaces to support continued learning and peer support.
  • Communicate participant program status updates to participants and FRO team members regularly to support participant success.
  • Track Cohort workshop attendance, homework completion, counseling sessions, and challenges to support high cohort completion rates.
  • Oversee the tracking, management, and arrangement of constituent support, such as transportation resources, childcare, food vouchers, etc.
  • Oversee general donor outreach procedures to secure general donations as needed to support cohort success.

Cohort Coordination and Administration

  • Work closely with the Program Manager, Executive Leadership Team, and Clinical Team to ensure programmatic success.
  • Learn and engage in training on the pre-existing BLOOM Curriculum.
  • Be present at all in-person BLOOM Cohort programmatic events, including set-up and take-down (taking place on Weekday evenings and Saturdays).

General

  • Participate in recurring Free Root Operation trainings and staff wellness days on community violence intervention, trauma-informed care, social work, and wellness to support the delivery of services.
  • Submit weekly written reports to Executive Leadership summarizing participant progress, clinical insights, participant conflicts, and high-level roadblocks.
  • Maintain strict confidentiality of participant and staff information.
  • Support leadership with program planning and management as needed.
  • Build trusting, supportive relationships with cohort members.
  • Navigate sensitive and personal subject matter with professionalism and care.
  • Demonstrate strong organization, time management, and follow-through.
  • Represent the organization positively in community and partner settings.

Typical Schedule & Time Commitment

  • 2026 Cohort Cycle: March–November

  • Commitment to the mission and values of Free Root Operation, with a passion for community violence intervention and community empowerment.
  • Strong interpersonal and communication skills.
  • Strong organizational, project management, and time management abilities.
