Clinical Manager - Registered Nurse
Position Summary
The Clinical Manager is responsible for directing and coordinating day-to-day clinical operations, ensuring high-quality patient care, and maintaining regulatory compliance across all disciplines. This role requires an RN with a solid understanding of home health regulations, Medicare requirements, and accreditation standards to lead clinical staff and drive quality outcomes
Responsibilities
- Oversee and coordinate all clinical services, ensuring quality patient care delivery across all disciplines (SN, PT, OT, SLP, MSW, HHA)
- Supervise, mentor, and evaluate clinical staff including field nurses, therapists, and home health aides
- Review and approve OASIS assessments, Plans of Care (CMS-485), and clinical documentation for accuracy and regulatory compliance
- Manage patient caseload assignments, monitor visit utilization, and ensure timely completion of SOC, recertification, and discharge workflows
- Conduct case conferences, participate in interdisciplinary team meetings, and provide clinical guidance on complex patient cases
- Ensure compliance with physician order management, timely Face-to-Face encounters, and certification/recertification timelines
- Monitor patient outcomes, HHCAHPS scores, and quality metrics; implement corrective actions as needed
- Ensure clinical operations align with CMS Conditions of Participation (42 CFR Part 484), ACHC accreditation standards, and state regulations
- Participate in internal audits, survey preparation, and quality improvement initiatives
- Collaborate with the Compliance Officer and Administrator to address regulatory changes and maintain survey readiness
- Participate in on-call rotation to support after-hours clinical needs
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Experience with PDGM, HHRG, and CMS billing requirements
- Familiarity with ACHC accreditation standards and survey processes
- Proficiency with electronic medical records (EMR); KanTime experience a plus
- Experience with QAPI program management and performance improvement projects
