About the Role - SCaN Navigator (Youth Case Manager)

As a SCAN Navigator within our team that works on the Service Coordination and Navigation (SCaN) program, you'll serve as a dedicated youth case manager working with youth ages 14–24 who are involved in street activity or the justice system in Chicago's South Shore community. This position is funded by the Department of Family and Support Services (DFSS) through December 31, 2025 , with a strong possibility of renewal based on program funding decisions made before the end of the year.

The SCAN Navigator will provide intensive, hands-on support to help young people achieve goals related to education, employment, housing, and wellness. You'll work directly with youth to develop personalized plans, connect them to community resources, and provide ongoing mentoring and guidance. This field-based position requires strong relationship-building skills, flexibility, and a commitment to trauma-informed, youth-centered care. In this role, you will:

• Develop individualized achievement plans with youth based on their goals and strengths
• Provide mentoring and life skills coaching to promote emotional and social growth
• Coordinate services across education, employment, housing, and mental health systems
• Make community referrals and support youth in accessing resources
• Build relationships with youth, families, schools, mentors, and legal stakeholders
• Track and document services in internal and funder systems (e.g., CitySpan, SDS)

Who we ' re looking for

Required:

• High School Diploma/equivalent with 3+ years of related work or lived experience
• Strong communication, organization, and time management skills
• Valid driver's license (IL, IN, or WI) and reliable transportation with insurance
• Ability to pass a background check and drug screening
• No history of child abuse or neglect

Preferred:

• Bachelor's Degree in a human service field
• Experience working with youth who have experienced trauma
• Knowledge of strength-based, youth development practices
• Familiarity with community resources and case management
• Comfort using digital tracking systems (e.g., CitySpan)

Additional Information

• Full-time position - flexibility for evenings and some weekends is required
• On-site at 1818 E 71st Street, Chicago, IL with local travel required via personal vehicle

Compensation and Benefits

• Salary: $39,000–$45,000/year, based on experience.
• Average of 17–25 days PTO + 9 holidays
