Housing Navigation & Case Management Specialist (Bilingual/Spanish)

They offer comprehensive HIV care, harm reduction, and outreach programs grounded in equity, compassion, and community empowerment.

We’re seeking a compassionate and resourceful Housing Navigation & Case Management Specialist to help individuals and families experiencing housing instability. In this role, you’ll provide personalized case management, crisis prevention, and housing navigation support while building strong community partnerships to expand housing opportunities.

Location : Chicago, IL

Salary : $50,000-$55,000

Key Responsibilities:

• Conduct client intakes, needs assessments, and individualized care plans
  
• Provide referrals and linkages to housing, healthcare, and supportive services
  
• Deliver crisis prevention and intervention for unhoused and at-risk clients
  
• Advocate for clients with landlords, housing providers, and agencies
  
• Facilitate financial literacy, budgeting, and credit management workshops
  
• Build and maintain strong partnerships with landlords and community organizations
  
• Document services, track progress, and ensure high-quality case management

Qualifications and skills:

• Associate’s degree in Social Services (Bachelor’s preferred)
  
• 2+ years Human Services or Case Management experience (required)
  
• Knowledge of HIV care and supportive services (preferred)
  
• Cultural competence with African American, Latino, and LGBTQ+ communities
  
• Strong organizational, time management, and facilitation skills
  
• Computer literacy and reliable transportation with valid driver’s license
