Physical Therapist
Home Health Physical Therapist - Zion, IL
Join our client's exceptional Home Health team as a Physical Therapist and bring your skills directly to patients' homes. You'll assess and implement customized therapy plans to help patients improve mobility, independence, and quality of life.
Responsibilities:
- Provide in-home PT assessments and create personalized treatment programs
- Monitor and document progress in coordination with the care team
- Educate patients and families on therapy routines and safe equipment use
- Supervise PT assistants and collaborate with other providers
Schedule:
- Monday-Friday, flexible daytime hours
- 1 weekend every 6 weeks and 2 holidays annually
Travel: Local travel required throughout Zion and nearby areas.
