The Grants Manager oversees the full life cycle of Tapestry 360 Health’s foundation, corporate, and government grant portfolio—from prospect research and proposal development to compliance, reporting, and stewardship. This position plays a critical role in securing and managing external funding that supports organizational sustainability, new initiatives, and community impact.

The ideal candidate is a strong writer, detail-oriented project manager, and relationship builder who can navigate both the technical and strategic aspects of grants management in a fast-paced healthcare environment.

Essential Duties and Responsibilities:

Grant Development & Strategy

• Identify, research, and prioritize prospective grant opportunities from government, foundation, and corporate sources.

• Lead and coordinate proposal development—including narrative writing, logic models, outcome measures, budgets, and required attachments.

• Collaborate with program, finance, and data teams to align proposals with strategic goals and community needs.

• Track submission timelines, ensure timely delivery, and maintain a competitive proposal calendar.

Grant Compliance & Management

• Serve as the central point of coordination for awarded grants, ensuring all activities meet funder and federal (2 CFR 200) requirements.

• Monitor progress, expenditures, and deliverables; coordinate internal reviews and updates with finance and program leadership.

• Lead the submission of narrative and financial reports, ensuring accuracy, timeliness, and alignment with funder expectations.

• Prepare for and participate in audits, site visits, and other compliance reviews.

Data Tracking & Reporting

• Maintain an accurate and current grants management database and reporting system.

• Develop internal dashboards to track award status, revenue, outcomes, and renewal cycles.

• Partner with the data team to ensure reporting aligns with HRSA and other funder performance measures.

Funder Relations & Stewardship

• Manage ongoing funder communication and relationship stewardship.

• Coordinate thank-you correspondence, recognition efforts, and funder updates.

• Support cultivation activities, site visits, and events with current and prospective funders.

Internal Collaboration & Process Improvement

• Build cross-departmental collaboration around funding opportunities and performance reporting.

• Develop and refine standard operating procedures (SOPs), templates, and internal workflows to strengthen grants management capacity.

• Provide training and technical assistance to staff on grant proposal development and compliance best practices.

Qualifications:

Education & Experience

• Bachelor’s degree required; advanced degree (MPA, MPH, or MBA) preferred.

• Minimum 5 years of progressive experience in grant writing and management, with at least 2 years in a healthcare, public health, or nonprofit setting.

• Proven success securing federal and foundation grants exceeding $500,000 annually.

• Familiarity with HRSA, CDC, SAMHSA, or similar federal grant mechanisms preferred.

Skills & Competencies

• Exceptional writing, editing, and storytelling abilities.

• Strong financial literacy and experience preparing and monitoring grant budgets.

• Knowledge of Uniform Guidance (2 CFR 200) and nonprofit compliance standards.

• Excellent organizational, analytical, and time management skills.

• Ability to manage multiple deadlines and projects simultaneously.

• High emotional intelligence and ability to work collaboratively across teams.

• Proficiency in Microsoft Office Suite and grants management platforms (e.g., GrantHub, Fluxx, Salesforce, or similar).

Salary and Benefits:

Compensation: The annual salary range for this position is $75,000 to $115,000, based on experience and qualifications.

Benefits: Tapestry 360 Health offers comprehensive benefits, including medical, dental, vision, retirement savings, paid time off, professional development opportunities, and eligibility for the federal Public Service Loan Forgiveness (PSLF) program.

Tapestry 360 Health is committed to equitable and transparent pay practices. In accordance with the Illinois Pay Transparency Act, we are disclosing the full salary range for this position. This range represents the potential compensation for the role based on experience, tenure, and performance over time.

Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget. Salary progression is evaluated regularly to support professional growth and retention.
