Job Description

Candidate will:

• Evaluate, plan, and provide treatment programs for patients receiving occupational therapy
• Treat patients with disabilities, disorders and injuries to develop or restore function in daily living tasks maximizing performance
• Provide a variety of health care services centered around the unique and individual needs of each patient according to the plan of care
• Meet the needs of all customers including patients, significant others, physicians, and other staff
• Work in multi-disciplinary teams in order to deliver a full range of services and participate in team based continuous improvement.

Qualifications

• Current IL Occupational Therapy Assistant Certification
• Current BLS certification
