How you will make an impact:
Join our team as a Social Services Coordinator (RN) and make an impact by coordinating psychiatric and psychosocial services to support comprehensive, person-centered care. You will assess resident needs, provide counseling, advocacy, discharge planning, and connect residents and families to vital community resources.
As part of our interdisciplinary team, you will play a key clinical role in behavior management, social engagement, and family communication. You will encourage resident participation in both individual and group programs, facilitate family involvement in care plans, and assist residents with legal and healthcare documentation to ensure seamless transitions and continuity of care.
Accurate and timely documentation of all social service plans and progress notes will be essential, reflecting goals and interventions that support the overall wellbeing of each resident.
Schedule:
This position is full-time, Monday through Friday (8:00 AM – 4:00 PM), with 1 weekend per month on-call responsibilities.
What you will need:
- Active Registered Nurse (RN) License required.
- Experience in geriatrics required.
- A minimum of 3 years of experience in a skilled nursing or long-term care setting, with a strong understanding of resident care, discharge planning, and interdisciplinary collaboration preferred.
- Strong communication, organization, and problem-solving skills.
